    1 Create Incident from Portal Login to ServiceNow at https://utatest.service-now.com/selfservice using netid and password.  
    Click "Report an Issue".  
    Fill the form.   
    Select What application has a problem.  
    What type of  the problem is.  
    Type Description.  
    Click the Submit button.
      incident is created        
    2 Create Incident from ITIL Provider Side   Login to ServiceNow at https://utatest.service-now.com/ using netid and password.    
    Under Incident tab,  click Create New.    
    Fill the form.     
    Select What application has a problem.    
    What type of  the problem is.    
    select the team to which to assign the incident to and select assigned person to.  
    Type Description.    
    Click the Submit button.
    incident is created        
    3 Resolve Incident Login to ServiceNow at https://utatest.service-now.com/ using netid and password.    
    select incident after clicking all under incident tab. 
    Select assigned to. 
    Click Closure tab
    select closure code
    enter Closing notes
    click Resolve button.
      incident is resolved            
    4    Parent & Child Incident
